In the Unholy Lands the gods of men are all but forgotten. Long ago their great cities were destroyed by Balaak, the Abomination Who Ascended, and his inhuman followers. The race of man has become little more than small groups of wandering nomads who fear to settle in one place too long lest they be destroyed by Balaak’s children.
Denied divine healing due to their lack of faith in the gods who failed their ancestors these people have survived through cunning and by necessity have substituted the mundane for the miraculous.
Taking the flesh of trolls, who have a natural regenerative ability, the wise men learned to treat it with certain acids and herbs which dilute its regenerating abilities. Ingesting only small amounts after being wounded causes the flesh to mend quickly and vigor to return to the weary.

I also quite like the implied difficulty in getting hold of it - you must first convince a troll to part with some of his own flesh (this is not going to be easy)

It would be funny to see though, convincing a troll to part with this flesh of his, then using it in battle against him to obtain more.
A society with very powerful members could even engage in "Troll farming."

About the shortness, sometimes ideas don't need a long writeup to explain them. As far as I am concerned, the short length of the writeup does not diminish the coolness of the idea.

Solid. Could use a little fleshing out. Maybe the preserved skin of a LIVING troll would be best - after all I doubt the flesh would be doing much regenerating if the troll is flayed after death. I like the mention of alchemy, but it could use a little fleshing out - I want to know more about the process of making it. Aside from a little acid, you didn't say enough about it to quench my curiousity. Could be something good - perhaps I'll come back to it once you extrapolate?

Meanwhile, if alchemy can distill the essences of living being to emulate their characteristics, your standard fantasy ecology offers lots of (evil) possibilities: need darkvision? A potion with the distillate of some underground race

As alchemy was very much a magic and emotional take of chemistry. I don't know if this is alchemy, this seems more like bringing an element of Sci-fi into the fantasy setting. Which certainly is fine in some settings, but this seems to be based on a clear vanilla D&D troll. I would be interested to hear what this community has to say about the realism, physics and chemistry versus the themes of a game. For example clever players may use mend spells to take the natural cracks out of a material such as flint to make it super hard and and sharp. Does this fit with the theme of the game.
At anyrate nice idea, simple, compact back story and easy to slide into a game. You stick in some other animal if you don't like trolls, hydra rations or something.
