If meant for a girl, it is a beautiful silver bracelet, if for a man, it takes the form of a wristwatch with a metal strap.
History
When the De Madden Company decided to really get to grips with The Children of Ma-O, because of the damage it's activities were doing to Company interests rather then the loss of life, they soon discovered that the cult was perfectly happy to murder anyone who acted as a witness against them , and their familes too. And every murder of a witness was a victory for the cult and made it harder to get people to testify.
So they created a Company funded witness protection program, so that witnesses could be renamed and moved to a new place to avoid the cult's assassins. However, not everybody in hiding wanted to stay in hiding. Some missed their families and friends, others could not settle, and a few felt it cowardly to hide and openly dared the cult to do it's worst. Such people generally did not last very long.
To stop it happening again, and to help their secret agents undercover in dangerous situations, they created the Masking Bracelet. Once wearing this, if somebody in hiding was about to blow their cover, it would mentally send them a warning. In most cases that would be enough, but for those who tried to blow their cover, it overrides them and shuts them up, stilling their tounge and forcing them to live the life assigned to them..If they continue, it is a battle of wills between them and the Bracelet.
It has a spell that prevents it being taken off, as it fuses to the wrist as if it is part of the skin. And if pulled or cut, one of it's defences is that it makes the wearer feel pain as if a bit of their own skin was being pulled off or cut.
Most are forced to stay undercover, but if the Bracelet finds it is clearly losing the struggle or is in danger of being cut off by the wearer or another person, it has three final defences. First it sends the person a vision of his or her fate if he or she is uncovered. If that does not work, it casts a short term sleep spell. Finally, if nothing else works, it injects the wearer with a powerful poison that will stop their heart and kill them within seconds.
Magic/Cursed Properties
Someone in hiding wearing this bracelet will find it mentally very hard to tell anybody their true identity in any way. The Bracelet cannot be taken off either. It is meant to be used to protect a witness, but it can also be used for kiddnapping, as a punishment for somebody, or just to help anyone undercover as part of an undercover operation.
The bracelet also casts a glamour spell over the wearer to make them seem to be a different person in looks and (rarely), in gender. If the gender appears to be changed, it changes the voice too to make it fit. Gender changes are not often done however, unless as part of a punishment.
Could also be used for kidnapping or human trafficking. I see it more useful for that than forcing somebody to stay safe. A prince could be kidnapped or a rich merchant abducted and paraded around most anywhere and if he ever tried to blow his cover this would stop him. Could be used for any number of scenerios like that.

Simple. Basic. Lacking of too much useful explanation. The modern title is jarring for a fantasy item (unless this is marked urban fantasy or magipunk or some such).
It's not a bad sub, actually. And considering that it is derived from Cheka's own world of Acqua which incorporates certain modern elements, he can call it pretty much whatever he thinks is suitable as long as it has some basis in Acqua. Nothing wrong with the explantion either. It's supposed to stop under-cover agents from exposing their true identities or turning traitor. Sounds pretty straight forward to me.
Its okay, though you refer to it in the main as the Witness Protection bracelet, while in the text it is referred to a the Masking bracelet. I think the second title addresses Moons concern with the modernity.
Now, what keeps them from simply taking it off?
An item, that just had to be invented, it goes well with your theme of transporting the modern into the fantastic.
The last set of the powers though, seems really like an overkill. Even if they reveal themselves, what good are the witnesses if they die?
It's an act of revenge on those so ungreatful as to want to reveal themselves despite all the trouble in hiding them.

Great seed, I like it but its quite simple.
Decent item all said but I could only see high profile witnesses that are worth a lot to the defense/prosecutor would actually use them. Most would be able to last through the trial, it is after that which probably becomes an issue.
