This really comes from an ancient Chinese novel. Imagine during the PCs' travels, they come to a place where the gender roles are completely reversed. What adventures will the PCs encounter in such a place?
A healing potion which if drank by males (or women in male clothing) causes a short term (1 d6 hours) urge to dress in girl's clothing due to the high levels of estrogen in it.
Consider the idea of spontaneous genesis of life, as used to be belived. What strange kinds of life might arise from modern substances? What kind of vermin would a landfill produce, or an oil spill on the ocean?
A sentiment weapon that is very impressionable in terms of what is right and wrong, akin to a child. Currently, being ownerless, it is not very powerful. However, once it has found an owner, it can provide significant boosts to the wielder in an area or areas the wielder most desire. What adventures will this sentiment weapon go through? How will it develop itself in terms of power and personality?
A ghost is haunting someone with a secret-it\'s not that he murdered the person that became a ghost, it\'s that he went into hiding with a new identity to avoid being murdered, whilst the ghost wants him to join her in the afterlife. The ghost is getting stronger and more dangerous each day. Enter the PCs.
An orc male who dislikes the hard warrior life, has disguised himself as an orc female and is learning how to become a Shaman. If uncovered as a male, his fellow orcs will be disgusted enough to kill him.
A fantasy version of the Klan, where it's "Grand Wizard" really is a mage, and a dangerous one at that. Instead of hating blacks most of all, they hate non humans most of all (although they are no friends of black people either and certainly don't let them join.)

Magic is addictive, small spells are like cannabis in addictiveness, major ones are like cocaine or herion. Many countrys ban even cantrips as a result.
When this is used, the user is full of static electricity, which will discharge itself onto the next living thing that he or she touches, causing minor to moderate injury.
Like a Binding Oath Ring but it stops anyone under witness protection from revealing his or her true identity to anyone. I might make this into a submission one day.
The Mad Pope is a wandering mercenary. He is very well deranged as he considers himself to be the pontiff of the dominant faith. His robes are tattered, his mitre has seen better days and there are surely lice in his long ratty beard. What sets him apart from most addle-pated would be holy men is that he has armor under his robes and carries a large crossbow and several one-handed swords. While many would discout him as just another lunatic, for some reason, he inspires others around him and has demonstrated the ability to lay on hands and heal the wounded.
People argue about the true form of the beasts known as "Bloodtouches", since they seem to take dozens of forms. Unfortunately, their bite's effect isn't in doubt. Anyone bitten by the bright crimson beasts rapidly loses all hair; their skin color shifts to bloody red. Many people fear those under this curse; villages of blood-skinned outcasts huddle in the wilds.
A planet (or city, valley ect) where those who enter witness protection can escape old age and death itself, so they can live for centuries without aging-provided they maintain their cover perfectly. If they are outed by anyone or ever utter their true name, they age the number of years they have not aged since the day they entered witness protection (which might mean that they age centuries in seconds and crumble into dust and bone.)
A group of wizards who will give others temporary magical powers, but the price is high and those who can't pay or won't pay get very badly hurt or even killed.
Elven prison sentences even for small offenses seem very long to humans, but this is not because their rulers are draconian, but because elves live so long that a six year sentence, for example, is like a six week one in human terms.Humans in Elven countries are well advised to behave themselves
The gold hoops and rings that sailors wear are to throw into the sea if there is a storm. These are bribes to the sea gods to let them live through the storm.
Once a year, the king steps down from his throne, and lives as a peasant for one day, the next day is the anniversary of his coronation, and it is celebrated by a reenactment of the ceremony. This is doubly beneficial, firstly, because the people are able to have the grand parade that always precedes the ceremony. Secondly, because the heir to the throne is given a day on the job every year. Although, apparently, the thought has not yet come to one of the kingdoms enemies, the assassination of the king during the renewal coronation would be a moral, as well as a political blow to the kingdom.
