Golden coins that turn to lead within a few hours of being taken from their place of origin, causing a lot of trouble for whoever spends them.
It seemed like a great place to camp. The clearing was good sized and sheltered from the wind. The brook just a few feet away. There is a natural hallow to keep the horses.
Then the night came.
It was like it became a different place. The temperature dropped. The wind, which does not seem to disturb cloth, almost cuts through you like an arctic wind. No one can sleep, as the soft ground has turned hard. The horses are uneasy. The Bats are flying over and stopping in the trees.
And then there is the eyes. There are glowing eyes just inside the tree line watching your group. The mages and clerics can detect nothing, but there is still something there.
(yet there is nothing at all... The Darkness will do nothing unless the players do something to it. And even then it will all seem to be a conincidence.)
Of course, in the morning, it all becomes sweet and light.
The PCs have been traveling for a long time down the same stretch or round with no sign of anything suspicious. As they cross over to the next hill they see a gigantic splater of blood with a bleached dragons scull in the center of it. It the dragon skull's mouth is a tattered backpack with something wiggling inside of it.
What if when a character died and the price to bring him back into the world was to allow an evil or good version to be placed in the world with the sole purpose of killing the character that just got its life back? The copycat character would die apon completeing his mission.....or would he?
An African tribe called the Ik throws their children out of their homes once they turn 3. They are left to fend for themselves with no help from their parents at all, and to survive, form groups with others their age. These groups only last a few years, and every so often the individual will join a completely different group.
Teenagers who run away from home to live in the wilderness may become feral. As they become more and more feral, they gain some supernatural powers (a la Peter Pan - ability to fly, things they imagine come true - possibly quite powerful), but at the expense of their humanity and ability to speak, comprehend complex subjects, etc. Maybe they have a leader: the most feral of them all.
A regression into a fantasy childlike state.
There was a Beggar he was starring at me as if he is going to kill me but as I get nearer and nearer I found out his resisting something I don't know what but his really resisting something something un explainable and then I saw a knife on his hand and I though his going to kill me but when he raises the knife and then he stoped and said "run child" that I knew his resisting himself' his blood thirsty self, to stop, to stop killing people at the other day when I go were he was I saw him lying DEAD! and I saw a knife stabed strait throught the heart and that knife was the one he tried to stab on me last day and I knew he KILLED HIMSELF because its the only way to get out of his problems...
A small group of men stares intensely at something. Coming nearer, you see puppies, obviously still young, as they are small and only learn to walk. Cute as they are, the men look serious and exchange a comment here and there, making their mother nervous. But one man keeps her somewhat calm, while looking with others on the little ones.
The puppies are a completely new dog breed. The breeder hopes for better characteristics, the other men are mostly hunters and other dog-fanciers, that were called or came from own interest. If all looks well, they will have business. But now, they are just watching.
There is a place where there are living dinosaurs but in miniture. So there are foot long Tyranosaurus Rex's and four foot long Brontosauruses, for example.
Beware of towns afflicted with Time Cancer.
For every second you progress through time, an infected town goes back 48 hours. Old buildings slowly become new, then incomplete, and finally just a frame and foundation, giant old trees turn to saplings, birds hatch into eggs.
one must not linger in a chrono-cancerous village for too long, before you know it you'll be a fetus. Tme cancer in unstoppable, no cure or treatment is knwn, it eats through the past until the inhabitants of a village turn into primordial protein ooze

The creation of intelligent magical weapons is not an exact science and there are occasional failures, as such think of causing a little more stress to your players by "rewarding" them with an insane magical weapon
Agrophobic, refuses to come out of the scabbard without a strength check to draw it.
Haemaphobic: afraid of blood, fight at a penalty against any blooded creatures but really good against undead, elementals etc
Schizophrenic, not just one mind in your sword, lets have half a dozen minds in there, all different and bound to cause confusion
the possibilities are endless
From an episode of Justice League Unlimited:
The God of War creates a giant, unstoppable robot (golem, animated armor, etc.) as a weapon for one side in a civil war; his purpose is to prolong the war and create suffering and war among humanity.
Quote from Ares in that episode, which would go well with this idea: "That's all you mortals are good for! To fight and fight and fight until there is nothing left but charred land and blood and bones, and to end it all and then start again with the next generation!"
A nifty little encounter appropriate for any powerful sorcerer/magically adept monsters lair.
When the party have penetrated to an appropriatly impressive level/room they encounter The Door, this door is a vessel through which the Sorcerer/Monster can safely work against any intruders, the door is heavy duty iron bound studded oak wrought with craft skill and magic, set in the middle is a crystal orb, once the party get within a preset distance the orb flickers briefly as the door warns its "master" of the intruders, the master can then effectivly possess the door itself, this means that all movement the door is capable of (i.e. opening and closing) come under the control of the master, the master can also cast spells from the door as if he were there himself, all the while any damage that is inflicted in return merely damages the door, this will in effect ruin any of the parties chances of surprise, allow the master to assess the parties abilities while remaining safe and sound and finally will probably cause the party to waste some of their juicier items/spells on a chunk of wood and iron, and if the master happens to have a deadly spell or three it could also reduce the size of the party,
Of course if you wanted to be really nasty you could have the door open to admit one person and then slam shut on the second person (squish) whilst blasting away with every spell in the mastrs repetoir
When someone befriends an animal, s/he obtains the ability to unite their souls together and take on the form of that animal as well as its abilities. Magical Fiends: When a mage or wizard casts a spell and draws more energy than s/he needs Magical Fiends are drawn to that spot and they feed of the extra magic as long as they can. When someone's magic is being fed off of they can't cast spells as well and they feel weaker. The only way to remove a magicial fiend is for it to be drawn away by someone else's power or to go to the very spot you first cast the spell and kill them
The PCs discover a large white ball in the branches of a tree, or in a niche in a cave. Unless magically examined, or by a PC with a specialty in insects, it is a plain, smooth white ball. After a short time, it hatches and releases hundreds of spiders, cockroaches, some other nasty bug
Near a major city, a mirror image of the place has appeared. This strange double is infested with hostile insectoid humanoids that appear to match the city folk in numbers and armaments. The humans’ diviners have determined that the secret of the city’s appearance can be found within the double’s college of magic, so heroes are needed to explore within this mirror city and discover what has drawn it from its distant realm. There isn’t much time to spare, as the bug folk find their mammalian counterparts unnatural and blasphemous, and plan to destroy them.
